MEDICATION PROCEDURES FOR THE SCHOOL CLINIC

Beginning the 2016 school year ALL medication, including cough drops, must have a Medication Agreement signed by a parent/guardian and a physician in order to be administered at school.

Medication Agreements, Asthma and Anaphylaxis Care Plans, Outdoor Lab forms can be printed from the folders below. You may also come to the office for a copy.

Over~the~counter and prescription medications must be in the original package. Medication not in the original package will not be administered to your child. Prescription and over~the~counter medications require a parent/guardian and a physician signature on the Medication Agreement. The prescription must be properly labeled with the appropriate students name, name of the pharmacy, medication, and dosage clearly marked. Please check medications for current expiration dates and that it is age appropriate for your child.

All medication must be stored in the school clinic. Students should never be allowed to administer medication, including cough drops, to themselves. Students may not keep medication in their pockets, backpacks, or with teachers, unless it is indicated by the physician on the Medication Agreement and approved by the District Nurse. Refrigeration is available.

All medications must be delivered to school, or returned home, by a parent or guardian.

The Clinic can always use donations of:

  • Bandaids
  • Paper towels
  • Sandwich size baggies
  • Sweat pants/underpants/shorts/t-shirts
